What is Trauma-Focused Cognitive Behaviour Therapy?
If you've found your way here, chances are you — or someone you love — has been through something hard. Maybe the word "trauma" has come up, or a professional has suggested counselling, and you're trying to make sense of what that actually looks like.
This blog is for you. It's a plain-language introduction to the approach I use at Betta Days: Trauma-Focused Cognitive Behaviour Therapy, or TF-CBT.
TF-CBT is one of the most rigorously researched trauma treatment approaches in the world, and it's designed specifically to work with children, young people, and adults — as well as the families and caregivers around them. It's evidence-based, which means it's been tested across thousands of people and shown to genuinely help.
STARTING WITH THE BASICS
First — what is trauma?
Trauma isn't just about "big" events. It's any experience that overwhelms your nervous system's ability to cope — leaving you (or your child) stuck in survival mode long after the danger has passed.
This might look like: flashbacks or intrusive memories, anxiety, nightmares, emotional outbursts, shutting down, avoiding reminders of what happened, or struggling to feel safe in everyday situations.
Trauma lives in the body and the brain — not just in thoughts and memories. That's why effective treatment has to work on multiple levels.
THE APPROACH
So, what exactly is TF-CBT?
TF-CBT stands for Trauma-Focused Cognitive Behaviour Therapy. It was developed by Drs Judith Cohen, Anthony Mannarino, and Esther Deblinger, and is endorsed by leading health bodies internationally — including the Australian Child and Adolescent Trauma Alliance.
At its core, TF-CBT helps people understand how trauma has affected their thoughts, feelings, and behaviours — and gently builds the skills to process what happened in a way that reduces distress and restores a sense of safety and control.
Crucially, TF-CBT is not about forcing anyone to relive trauma. You always go at your own pace, with the therapist walking alongside you — not ahead of you.
WHO IT'S DESIGNED FOR
Who does TF-CBT help?
TF-CBT was originally developed for children and adolescents, but it has a strong evidence base across the lifespan. At Betta Days, I work with:
Importantly, TF-CBT involves caregivers wherever possible — because the people around a child matter enormously. Parents and carers often have their own parallel sessions, so everyone is growing together.
COMMON QUESTIONS
Things people often wonder
"Does my child have to talk about what happened straight away?"
Absolutely not. In fact, TF-CBT is specifically designed to do the opposite — we spend significant time building safety and skills before we ever approach the trauma content. Your child (or you) will always be in control of the pace.
"How long does TF-CBT take?"
Traditional TF-CBT is structured across roughly 12–25 sessions, but at Betta Days I work flexibly based on what each person needs. Some people move through quickly; others benefit from a slower pace. We check in regularly and adjust as we go.
"My child won't talk about feelings — will this work?"
Yes, and this is actually really common! TF-CBT has a rich toolkit beyond verbal conversation — including creative arts, movement, storytelling, and play-based approaches. I work with children who communicate in all sorts of ways.
"Is this suitable for trauma from a long time ago?"
Yes. Trauma doesn't have an expiry date, and neither does healing. Many adults come to therapy carrying experiences from childhood, and TF-CBT can be deeply effective across the lifespan.
